Liability Waiver Agreement

LIABILITY WAIVER AGREEMENT

As a student/participant of Blue Sage Studio, LLC, I hereby agree to the following:

 

1.  I understand that this class includes physical movements as well as an opportunity for relaxation, stress re-education and relief of muscular tension. As is the case with any physical activity, the risk of injury, even serious or disabling, is always present and cannot be entirely eliminated. 

   1. If at any time during the class, you feel discomfort or strain, gently come out of the posture, and notify your teacher. You may rest at any time during the class or end your session if necessary. It is important that you listen to your body and respect its limits on any given day.

2.  I, the undersigned, understand that yoga is not a substitute for medical attention, examination, diagnosis, or treatment. The classes (Yoga, Zumba, Pilates, etc.)  can be unsafe under certain medical conditions. If I have a medical condition, I agree to consult with my physician prior to beginning any activity/class. I hereby agree to irrevocably release and waive any claims that I have now or hereafter may have against The Blue Sage Studio, LLC and its staff members. 

3. My signature further acknowledges that I shall not now or at any time in the future bring any legal action against teacher at Blue Sage Studio; and that this waiver is binding on me, my heirs, my spouse, my children, my legal representatives, my successors, and my assigns. My signature verifies that I am physically fit to participate in classes and a licensed medical doctor has verified my physical condition for participation in this type of class. I am voluntarily participating to take this class. 

4. If I am pregnant or become pregnant or am postnatal, my signature verifies that I am participating in classes with my doctor’s full approval. I realize that I am participating in classes at my own risk.

5. I am 18 years or older. If I am not 18 years or older, a parent will sign on my behalf. 

6. All sales for sessions are final. I have read and fully understand and agree to the above terms of this Liability Waiver Agreement. 

7. I hereby certify that I have read this document; and, I understand its content. I am aware that this is a release of liability as well as a contract and I sign it of my own free will.
